Instructions: Begin by downloading the template below and then populate with your membership details. For each player, first name, last name, birthdate and at least one email is required. When your file is uploaded, we will add each player to your membership. Once completed, you will be able to email your players via the membership section. Your users will gain access to their players once they create an account.
